“Lean,” A New Hip High

It’s more than a drug; it’s a culture. It’s what’s known on the street as "Lean," a highly addictive cocktail of cough syrup, cold medicine, alcohol and candy — so potent it makes you "lean" over when high.

The drug first began to get attention a few years ago, when a popular Houston DJ overdosed on it. At that time, it was easy to make and easy to get, says Ron Peters, a professor at the University of Texas School of Public Health. "As far as across Texas, across also the southern part of the United States, estimates have shown that it to be at one time a pretty common drug of choice amongst kids…anywhere from ninth grade all the way up to young adults," says Peters.

Authorities have recently tried to crack down on the drug, but while the crackdown has made it harder to get, it’s also made it easier and more profitable to traffic.

"A year and a half to two years ago the price for a pint of codeine promethazene cough syrup was about $20 and was called a deuce, and for 16 oz, which was called a PT cruiser, would go for about $120 to $125," says Peters. "Now a pint of codeine promethazene would go for anything from about $250 all the way up to $350."

Pharmacists, doctors and drug dealers from other states are getting into the act, says Peters.

Terrence Kiel, a player for the San Diego Chargers, was arrested this week for allegedly shipping large quantities of prescription-strength cough syrup to Texas, where Lean is most popular. He is expected to enter a plea in court next week.

Lean is so popular that it produced a Houston-based rap called screw music named after DJ Screw, who overdosed on the drug. The music takes hip-hop beats and slows it down to a crawl, reflecting the sleepy, laid-back feeling the drug gives its users. It was even the inspiration for a Top 10 song by the Oscar-winning group Three-Six Mafia, "Sipping on Some Syrup."

Peters says the drug and the music have grown together. "One of the studies did show that a lot of children found out about what codeine promethazene was through the music," he says. "And some of them stated that their first time finding out about it was through the messages that were in this screw music."

But, Peters says, the music is a way to make parents and the community aware of what is happening. He instructs parents to "always listen to some of the music some of our kids listen to," he says. "Whether they like or not, it can give them insight into what’s happening in their kids’ lives and open up dialogue."

Purple drank is a slang term for a recreational drug popular in the hip hop community in the southern United States, originating in Houston, Texas. Its main ingredient is prescription-strength cough syrup containing codeine and promethazine. Cough syrup is typically mixed with ingredients such as Sprite soft drink or Mountain Dew and pieces of Jolly Rancher candy. The purplish hue of purple drank comes from dyes in the cough syrup.

There are numerous slang terms for purple drank, including sizzurp, lean, syrup, drank, barre, purple jelly, Texas tea, and Tsikuni.

Houston, Texas producer DJ Screw popularized the concoction, which is widely attributed as a source of inspiration for the "chopped and screwed" style of hip hop music. Originally, the active ingredient of "syrup" was cough syrup containing promethazine and codeine. The concoction first gained popularity in the underground rap scene in Houston, where musician Big Hawk said it was consumed as early as the 1960s and 1970s, becoming more widespread in the early 1990s. Its use later spread to other southern states. Because of usage by rap artists in Houston, it became more popular in the 1990s.

In June 2000, Three 6 Mafia's single "Sippin' on Some Syrup," featuring UGK, brought the term "purple drank" to a nationwide audience. Three 6 Mafia's single "Rainbow Colors" featuring Lil' Flip pertains to the consumption of purple drank; the addition of a Jolly Rancher candy to a cup of purple drank creates a spectrum of colors, hence the name.

In 2004, the University of Texas found that 8.3% of secondary school students in Texas had taken codeine syrup to get high. The Drug Enforcement Administration reports "busts" involving syrup across the southern United States, particularly in Texas and Florida.

Notable deaths from use

Purple drank is confirmed or suspected to have caused the deaths of several prominent users. Respiratory depression is a potentially serious or fatal adverse drug reaction associated with the use of codeine, but mainly the danger lies in the much more potent and CNS-depressing phenothiazine-related antihistamine promethazine. This depression is dose-related and is the mechanism for the potentially fatal consequences of overdose: respiratory or cardiac arrest. As with most CNS depressants, mixing with alcohol greatly increases the risk of respiratory failure and other complications.

DJ Screw, who popularized the codeine-based drink, died of a codeine-promethazine-alcohol overdose on November 16, 2000, several months after the video to Three 6 Mafia's single debuted.

Big Moe, a DJ Screw protégé whose albums City of Syrup and Purple World were based on the drink and who has been described as having "rapped obsessively about the drug," died at age 33 on October 14, 2007, after suffering a heart attack one week earlier that left him in a coma. There was speculation that purple drank may have contributed to his death.

Pimp C, widely influential Port Arthur, Texas rapper and a member of rap duo UGK, was found dead on December 4, 2007, at the Mondrian Hotel in West Hollywood, California. The Los Angeles County Coroner's Office reported that the rapper's death was "due to promethazine/codeine effects and other unestablished factors." Ed Winter, assistant chief of the Coroner's Office, said the levels of the medication were elevated, but not enough to deem the death an overdose. However, Pimp C had a history of sleep apnea, a condition that causes one to stop breathing for short periods during sleep. A spokesman for the coroner's office said that the combination of sleep apnea and cough medication probably suppressed Pimp C's breathing long enough to bring on his death.

Other notable incidents

In September 2006, Terrence Kiel, a San Diego Chargers player, was arrested during practice for the possession with intent to sell prescription cough syrup for use in making the drink. Kiel was caught trying to ship a case of syrup to a friend via FedEx. Kiel was charged with two felony counts of transporting a controlled substance and three counts of possession for sale of a controlled substance.

On July 8, 2008, Johnny Jolly, a Green Bay Packers player, was pulled over in his car for excessive music. The officers found a Dr Pepper bottle in a holder next to two Styrofoam cups containing soda and ice. The officers said the cups and the bottle all emitted "strong odors of codeine." The case was dismissed at first, but charges were refiled in December 2009 after the Houston Police Department's acquired new equipment that allowed the police to test the evidence again. Jolly faced a possible maximum sentence of up to 20 years in jail, but as a first time offender he would be eligible for probation.

On July 5, 2010, former Oakland Raiders quarterback JaMarcus Russell was arrested at his home in Mobile, Alabama, for possession of codeine syrup without a prescription. He was arrested as part of an undercover narcotics investigation. Russell was booked into city jail and released soon afterwards after making his bail.

On June 11, 2013, just days after being robbed at gunpoint in San Francisco, rapper 2 Chainz was arrested at Los Angeles international airport on charges of possessing marijuana and promethazine and codeine, the primary ingredients of purple drank.


The most popular type of codeine syrup is promethazine-codeine, a prescription cough syrup. The active ingredients are codeine, a narcotic, and promethazine, an antihistamine. When taken in large quantities, both medications can lead to sedation and altered levels of consciousness. The inclusion of the antihistamine is intended to deter abuse, as doses higher-than-recommended can produce extreme somnolence, clinical weakness, and ultimately, fatal hypoventilation (inadequate breathing to sustain life). In lower doses, the antihistamine targets cold symptoms through reducing both swelling and vasodilation; it also acts to potentiate the opiate codeine.

Prescription cough syrups containing hydrocodone are also used to make the drink, though they are less popular. Songs like "Sippin' on Some Syrup" by Three 6 Mafia refer to Tussionex, a yellow cough syrup containing extended-release hydrocodone and chlorpheniramine (another antihistamine). Other hydrocodone-containing syrups such as Histinex HC, Hycotuss, and Hycodan may also be used, but Hycodan has added homatropine to deter abuse. Syrup also is made with over-the-counter cough syrups such as Robitussin DM, which contain dextromethorphan as the cough suppressant. Although dextromethorphan is used recreationally, it has dissociative effects as opposed to narcotic. Dextromethorphan is a synthetic morphine analog that has been on the market in the United States since the 1950s. It is a cough suppressant in small doses, but in large doses it can result in a disassociative state, with hallucinations, similar to that produced by PCP or ketamine.

Promethazine-codeine contains 10 mg of codeine and 6.25 mg of promethazine per 5 mL.

Some users report that the large amount of sugar in drank causes them to experience weight gain, tooth decay, and other medical symptoms.

Mentions in hip hop

In addition to its popularization in the music of DJ Screw and Three 6 Mafia, the mixture has been referenced in lyrics of other rappers. It is the subject of UGK's "Sippin and Spinnin" and "Purple Drank", as well as tracks by D12, Eminem, Lil' Wyte, Big Moe, Lil Boosie, Far East Movement, Paul Wall, Esham, Mike Jones, Kanye West, T.I., Rick Ross, Birdman, Future, Lil' Flip, Lil' Wayne, Ludacris, 2 Chainz, Playaz Circle, Fat Joe, Beanie Sigel, Project Pat, Chamillionaire, Lou Bega, French Montana, Kirko Bangz, Jim Jones, The Game, Slim Thug, Fat Pat, Frayser Boy, Gorilla Zoe, YC, Z-RO, Youngbloodz, 8Ball, Papoose, Drake, Jae Millz, Meek Mill, Mack Maine, Ace Hood, Juicy J, Gucci Mane, Plies, ASAP Rocky, Tech N9ne, Trae, Young Buck, E-40, Yelawolf, Schoolboy Q, Mac Miller and Ab-Soul, the latter of whom has crafted an ode to lean titled, "Mixed Emotions".

New Orleans rapper Lil' Wayne has publicly acknowledged his use of purple drank, and his lyrics frequently mentions drinking purple drank. In the Duffle Bag Boy music video he can be seen holding a Styrofoam cup with "RIP DJ Screw" written on it. In his freestyle to "Throw Some D's" on his mixtape Da Drought 3 he claims "I'm not a rookie, I'm a pro..methazine fiend" as well as stating "You know what's in my Styrofoam...what? S-Y-R-UP." He also mentions the substance in the track "Barry Bonds" from Kanye West's Graduation album, saying "My drink is still pinker than the Easter Rabbit," an overt reference to the color of the beverage. Wayne makes a similar reference in DJ Khaled's song "We Takin' Over." On March 15, 2013, it was reported that Wayne had been admitted to the Intensive Care Unit at Cedars-Sinai Medical Center in Los Angeles after being discovered "shaking uncontrollably" and "unconscious". Doctors found a high amount of codeine in his system and his stomach had to be pumped three times. He was said to be in critical condition. The rapper's associates were quick to deny rumors that he was near death, and he was released on March 18.

Commercial products

Several legal commercial products loosely based on "purple drank" are marketed in the United States. In June 2008 Innovative Beverage Group, a Houston, Texas-based company, released a beverage called "Drank." The commercial product contains no codeine or promethazine, but claims to "Slow Your Roll" with a combination of herbal ingredients such as valerian root and rose hips as well as the hormone melatonin. Similar "relaxation" or "anti-energy" drinks on the commercial market use the names "Purple Stuff", "Sippin Syrup", and "Lean".


These commercial products have been criticized for their potential to serve as gateways to the dangerous illegal concoction. At a mental health conference in February 2010, Dr. Ronald Peters, Jr., of the University of Texas Health Science Center said of "Drank": "They're taking the name, and they're trying to market it to young people." He described the beverage as "the worst thing I've ever seen on the street since the making of candy cigarettes."

Media speculation as well as internet buzz are openly querying if Trayvon Martin's purchase of Skittles and Arizona Watermelon Fruit Cocktail Juice on the night he died were just to satisfy a young man's sweet tooth or were they two of the three ingredients in a dangerous codeine-based concoction, as reported by the pop culture news portal on July 12, 2013.

Martin has been portrayed by his supporters as an innocent youth armed with nothing more than a bag of Skittles candy and a can of Arizona Iced Tea on the night he was killed by neighborhood watch captain George Zimmerman..

During the course of the trial, State Prosecutor John Guy made it official when he identified to the jury Martin's personal possessions found on his body by police, to include an "unopened bag of Skittles" as well as a "full can of watermelon-flavored drink," as reported by Central Florida News 13 on June 25, 2013.

Without citing reference sources, accuses Martin of admitting on his Facebook account as early as June 27, 2011 of being an abuser of a codeine, soft drink and candy beverage popularly known as "Purple Drank" or "Lean."

According to the hipster website, Martin asked a friend online, “unow a connect for codien?”

Martin went on to tell his friend that “robitussin nd soda” could make “some fire ass lean.”

He says, “I had it before” and that he wants “to make some more.”

The statements are credibility-weak without any source documentation cited, but that's exactly what the openly right-of-center weblog/news portal The Conservative Tree House has posted on their site via screen captures of the actual Facebook pages (click here to view).

As cited by the popular contemporary culture reference site Urban Dictionary, Purple Drank/Lean also goes by the street names of sizzurp, Texas Tea and liquid codeine.

Purple Drank is commonly used by Southern Rappers and "wannabe suburban teenagers" according to Urban Dictionary.

Also cited is that the intoxicant is "a mixture of Promethazine/Codeine cough syrup" and any given store-bought clear soft drink or juice.

To further enhance the sweet taste that appeals to the mostly youthful users, Urban Dictionary cites Purple Drank also has "a few jolly ranchers and/or skittles thrown in."

I can tell you when we were kids that NEVER happened. We were opening them as we walked out the door. Heck even now if my husband stops for skittles and a soda they are open before he can Drive the 5 minutes home. And since when does it take that long to walk a mile? Clearly he was doing something besides walking and it was sitting somewhere drinking his juice and eating his skittles. Martin was, evidently, a drug-crazed paranoid. He was, as Miss Jeantell's testimony revealed, a homophobe (literally: he was afraid of being raped by the assumed homosexual rapist, George Zimmerman). He told Zimmerman that, although the latter had had no problem up to that point, "[he did] now."

Trayvon Martin was a thug. He attacked, unprovoked in any meaningful way, George Zimmerman, and he intended to do him great bodily harm, if not kill him (which the evidence seems to point to). Martin deserved to die.

I heard his friend. His brother. What difference does it make anyway. Rather he was planning to do other drugs is beside the point. Trayvon turned around and went back to attack GZ who according to the reports I read was "less than 30 yards" from his own car. Even his "friend' who Trayvon was talking to on the phone has said that he was putting a "whoop a$$" on GZ. "Whoop A$$" = Self Defense! A poor defenseless kid didn't attack a grown man. A full sized almost adult, wanna be gang banger did. Look at the most recent picture of him. Yes he could be 17 but he could easily be 22 or 23. If he looked young for his age he could be 25 or older. Add a dark night, a hoodie, a mean look on his face and him banging your head into the pavement and tell me that you would think "He's just a kid." "I'll buy, if you'll fly" has long been the password for social drug sharing. There is enough evidence to convince anyone (except those whose minds are not already polluted by the left-stream media) that either young man was buying candy and watermelon cocktail for a sugar high. All the facts point elsewhere, although there is a "shadow of a doubt". That shadow, I belive, does not exist for George Zimmerman: he would be convinced that Martin was on drugs, just as he told the police dispatcher before Martin attacked him.

For those who review the entirety of the evidence, it is clear that Martin was not a choir boy. He was a thug and a drug abuser, as his liver tells us.

GZ had helped uncover a cover up where a cop's son beat up a homeless guy (a black guy, BTW) and I believe it ended up with at least one cop having to resign. No doubt, the inclination is to boycott Florida, stop conventions, to isolate Florida as a kind of apartheid state. ... Homicides have tripled since this law ["Stand Your Ground" ; not used in Zimmerman's defence] was enacted."

Correct Verdict

CHARLES BARKLEY: Well, I agreed with the verdict. I feel sorry that young kid got killed. But they didn't have enough evidence to charge him. Something clearly went wrong that night. Clearly something went wrong. I feel bad for anybody who loses a kid, but if you looked at the case and you don't make it -- there was some racial profiling, no question about it. But something happened that changed the dynamic of that night, and I know -- that's probably not a popular opinion among most people but just looking at the evidence I agreed with the verdict.

I just feel bad because I don't like when race gets out in the media because I don't think the media has a pure heart, as I call it. There are very few people have a pure heart when it comes to race. Racism is wrong in any, shape, form -- a lot of black people are racist too. I think sometimes when people talk about racism, they say only white people are racist. There are a lot of black people who are racist. I don't like when it gets out there in the media because I don't think the media has clean hands.

MARIA BARTIROMO, CNBC: I'm glad you made that point.

BARKLEY: Obviously I feel sorry that young kid got killed but just judging by the evidence, I don't think that guy should have went to jail the rest of his life. Something happened bad that night, obviously.

BARTIROMO: I like what the juror said, they both should have walked away. And if there is a shadow of a doubt, there is a shadow of a doubt.

BARKLEY: And let me tell you, Mr. Zimmerman was wrong to pursue -- he was racial profiling. I think Trayvon Martin, God rest his soul, I think he did flip the switch and started beating the hell out of Mr. Zimmerman. But it was just a bad situation. And like I said, the main thing I feel bad for, it gives every black and white person who is racist a platform to vent their ignorance. 

That's the thing that bothers me the most because I watched this trial closely and I watch all these people are television talking about it. A lot of people have a hidden agenda. You know, they want their racist views, whether they are white or black . . . .

BARTIROMO: The bias comes out.

BARKLEY: The bias, it definitely comes out. It was a bad situation, we all lost. And I feel bad for his parents. You don't ever want to see anybody lose a kid.
